À l'expiration de ce délai, les séances non utilisées seront considérées comme perdues et ne pourront donner lieu ni à remboursement ni à prolongation, sauf cas de force majeure dûment justifié (maladie grave, accident, grossesse à risque, etc.), sur présentation d'un justificatif.

Une prolongation exceptionnelle d'une durée maximale de 3 mois peut être accordée sur demande écrite formulée avant la date d'expiration du forfait.
